## Title: Case Manager – HIV Prevention and Care Coordination

About Tanzania Women Research Foundation (TAWREF)

The Tanzania Women Research Foundation (TAWREF) is a Tanzanian NGO established in 2010. TAWREF supports the government of Tanzania in expanding access to healthcare services, education, shelter, and economic opportunities for marginalized communities within and outside the Kilimanjaro region. The organization uses evidence-based approaches to deliver community-centered interventions that transform the lives of disadvantaged and vulnerable populations, including women, marginalized men, youth, children, and people living with disabilities.

Position Overview

TAWREF is seeking a competent, dynamic, energetic, committed, and experienced Tanzanian professional to serve as Case Manager for the Epidemic Control (EpiC) Project. This is a full-time position based in Moshi.

Key Responsibilities

  • Implement HIV prevention programs targeting at-risk populations in the region in collaboration with Community Engagement Officers and Biomedical Officers
  • Coordinate and monitor linkages of newly diagnosed HIV-positive individuals to care and treatment centers
  • Supervise all activities related to client retention in care for identified and diagnosed HIV-positive clients
  • Support clients who have missed appointments and are lost to follow-up to return to care
  • Ensure that all Navigation processes are established, conducted, and maintained according to Care and Treatment standards
  • Promote and monitor client-centered approaches during service provision to HIV-positive clients
  • Supervise and build the capacity of Peer Navigators through ongoing progress monitoring and support

Required Qualifications

  • Tanzanian national
  • Proven experience in case management and healthcare coordination
  • Strong knowledge of HIV prevention, care, and treatment programs
  • Demonstrated ability to work effectively with vulnerable populations
  • Excellent organizational and supervisory skills
