Job description
Job Purpose
To lead the reintegration of children in residential care facilities into safe and stable family environments. To take lead in the case management component of the District transformation project, working in partnership with Residential Care Institution social workers to ensure the transition of all children into family-based care. To ensure that the development and implementation of children's plans keeps them safe and secure, while designing and implementing prevention services in the community and other family-based alternatives to institutional care. To ensure that social work practice is effective, safe, and rigorous, and to mentor and supervise social workers in child care reforms.
Key Responsibilities
Case Assessment and Management
- Carry a complex child and family caseload, including children within the community, children in Residential Care Facilities, and care leavers
- Conduct initial screening and documentation of children's biographical information
- Lead assessment of children and families, including tracing activities
- Enroll children and families into the programme of case planning and assessment
- Collaborate with delivery partners to ensure post-reintegration support through active family support, parenting programmes, or referral to other service providers
- Conduct post-reintegration follow-up and review
Stakeholder Coordination and Communication
- Work in partnership with Residential Care Facility social workers and other partner organisations
- Provide leadership across all case management activities
- Maintain effective communication and representation of Child's i Foundation to all stakeholders
Reporting, Training, Records and Compliance
- Support social work practice standards and compliance requirements
- Mentor and supervise social workers in child care reforms
