Caring Habitat is a 40-room, purpose-built inpatial skilled nursing and rehabilitation hospital dedicated to complementing patient health outcomes through the delivery of robust rehabilitation programs.

Position Overview

We seek a compassionate and detail-oriented Registered Nurse who will assess, plan, implement, and evaluate patient care plans in collaboration with a multidisciplinary healthcare team.

Key Responsibilities

Patient Assessment and Care

• Effectively use admission and discharge protocols on the electronic medical record (EMR)
• Monitor patients' conditions and assess their needs to provide optimal care and advice
• Observe and interpret symptoms, communicating findings to the physician
• Create individual care plans in collaboration with multidisciplinary team members including medical officers, occupational therapists, physiotherapists, speech therapists, and dietitians
• Accompany inpatients to follow-up appointments

Clinical Procedures

• Perform routine baseline vital signs checks including temperature, pulse, respiration, blood pressure, SpO2, and fasting blood sugar (FBS) at shift start and end
• Provide wound care management
• Administer medications as prescribed by physician through oral, intravenous, intramuscular, and suppository routes
• Provide meal support, Percutaneous Endoscopic Gastrostomy (PEG) care, and assisted feeding for patients with dysphagia
• Perform tracheostomy care
• Provide resuscitation support including suctioning, oxygen administration, and nebulization as prescribed
• Perform cannulation and phlebotomy; monitor intravenous fluid administration
• Support medical officers in blood and specimen collection from patients
• Monitor and record intake and output charts
• Participate in routine nursing ward rounds with medical officers and clinical director as applicable
• Conduct patient handovers at bedside

Documentation and Safety

• Complete comprehensive report writing
• Maintain excellent safety and hygiene standards
