Health Coach at mDoc Healthcare

About the Role

As a Health Coach at mDoc Healthcare, you will be part of a dedicated team providing direct self-management support services to members facing chronic health challenges such as diabetes and hypertension. Through our innovative self-management model, you will deliver care both virtually and in-person, working across hub-based and field settings within the community. You will follow evidence-based guidelines for care while adapting them to meet each member's unique needs.

Key Responsibilities

• Provide direct virtual and in-person self-management support to members managing chronic health conditions
• Respond to member needs asynchronously and schedule one-on-one synchronous consultations
• Empower individuals to take an active role in their wellness by providing tools and support for lifestyle modifications
• Collaborate with virtual team members to help individuals meet their wellness and health goals
• Monitor member-level data and population-level trends to drive engagement and adherence
• Cultivate an environment that reflects mDoc Healthcare's values of respect for members and team colleagues
• Deliver optimal, member-centered care and experiences

Required Qualifications and Skills

• Data-driven mindset with ability to analyze member needs and population-level data
• Comfort with technology and remote working tools
• Ability to support individuals both remotely and in-person
