## Title: Registered Nurse
About the Organization
Caring Habitat is a 40-room, purpose-built inpatient skilled nursing and rehabilitation hospital designed to complement the health outcomes of patients through the delivery of robust rehabilitation programs.
Position Overview
We seek a compassionate and detail-oriented Registered Nurse who will assess, plan, implement, and evaluate patient care plans in collaboration with a multidisciplinary healthcare team.
Key Responsibilities
Patient Assessment and Care
- Effectively use admission and discharge protocols on the EMR
- Monitor patients' conditions and assess their needs to provide optimal care and advice
- Observe and interpret symptoms and communicate findings to the physician
- Create individual care plans for patients in collaboration with other team members (medical officers, occupational therapist, physiotherapist, speech therapist, dietitian, etc.)
- Accompany in-patients on follow-up appointments
Clinical Procedures
- Perform routine baseline vital signs checks (Temperature, Pulse, Respiration, Blood Pressure, SpO2, Fasting Blood Sugar) at shift start and end
- Provide wound care
- Administer medications as prescribed by physician (oral, intravenous, intramuscular, suppository, etc.)
- Provide meal support and assist with Percutaneous Endoscopic Gastrostomy (PEG) and assisted feeding for patients with dysphagia
- Provide tracheostomy care
- Perform resuscitation procedures, including suctioning, oxygen administration, and nebulization as prescribed
- Perform cannulation and phlebotomy; monitor intravenous fluids and support medical officers in collecting blood and other specimens
- Monitor intake and output charts
- Participate in routine nurse ward rounds with Medical Officers and/or Clinical Director
- Conduct bedside handover when taking over patient care
- Complete detailed report writing

- Maintain excellent safety and hygiene standards
