Clinical Hub Ambassador - Registered Nurse or CHEW at mDoc Healthcare

About the Position

Join our team as a Clinical Hub Ambassador providing direct virtual and in-person self-management support services to members through our innovative self-management model. You will work collaboratively with a virtual team to help individuals meet their wellness and health needs while following evidence-based care guidelines adapted to each member's unique circumstances.

Key Responsibilities

• Deliver direct self-management support services to members both virtually and in-person
• Oversee daily operations of the health hub, ensuring a welcoming and efficient environment
• Drive subscription uptake and member engagement
• Guide individuals through their care journey at NudgeHubs, encouraging them to take an active role in their wellness
• Provide tools and support to help members make positive lifestyle modifications
• Collaborate with virtual team members to address individual and population-level health needs
• Monitor member data to identify needs, respond to them, and develop strategies to drive continued engagement and adherence
• Cultivate an environment reflecting mDoc Healthcare's values of respect for members and team members
• Provide optimal, member-centered care and experience

Work Schedule

In-person presence required 6 days per week, including Saturday (half day).

Required Qualifications

• Registered Nurse (RN) or Community Health Extension Worker (CHEW) credential
• Strong data-driven mindset with ability to analyze member needs and population-level metrics
• Comfort with technology and ability to work both remotely and in-person
• Responsive and adaptive approach to member care and changing circumstances
• Initiative and willingness to learn and grow with a startup organization
• Passion for helping people improve their health and wellness
• Excellent communication and collaboration skills
