Primary Objective

Nutrition situation monitoring and support across MdM-supported health facilities in the designated woredasÖ

Key Responsibilities

• Conduct health facility capacity assessments and institutional mapping of TFP programs in collaboration with Woreda health office and project team
• Map community-level actors and develop referral pathways
• Provide technical support for community-level nutrition activities
• Conduct Woreda-level monitoring and evaluation of nutrition interventions
• Update weekly tracking tools to monitor programme screening data on acute malnutrition
• Collect health facility-based disease surveillance reports and update morbidity reporting tools for assigned health facilities
• Participate with MEAL team in supervision of data collection activities
